Health Insurance - United Healthcare

| Health Insurance is available to all full-time employees. Two plans are available from which employees may choose. An additional benefit is available to include a prescription co-pay plan as well. Utilizing an in-network physician allows for the highest level of benefits and does not require a referral under the PPO plan. Out-of-network coverage is also available for participants who find it necessary to visit doctors not included
in-network. |

| Dental Insurance is available to both part-time and full-time employees. Two plan options are available from which to choose. Both plans include orthodontia procedures. Any licensed dental provider can be seen, however, in-network services are provided to participants at lesser
fees. |

| Short Term Disability – Union Security |
|
| Short-term disability is available to both part-time and full-time employees. Participant can choose the benefit up to 65% of employee’s monthly income. Plan coverage to include sickness, accident, or pregnancy. There is a 14 day elimination period and benefits are payable up to 90days |

| Long Term Disability – Union Security |
|
| Long-term disability is available to both part-time and full-time employees. Participant can choose the benefit up to 60% of employee’s monthly income. Plan coverage to include sickness, accident or pregnancy. There is a 90 day elimination period and benefits are payable up to 5 years |

| 403b Retirement Plan – The Principal Group |

| There is no minimum contribution required to participate in plan. The Health Center at Standifer Place matches 50% of employee’s contribution up to $1,250 annually. Employees are vested after three years of service. |

| The Health Center at Standifer Place offers $10,000 company paid life insurance for full-time employees along with an additional $10,000 accidental death and dismemberment benefit. |
